The tires on our Sunseeker were included in the recall. I called a local truck shop that sells Michelins. They had the forms on hand but were out of the tire in that size. It took a few days to get the tires.
The forms were multipart (think carbon copies), so they couldn't be emailed and printed. The shop took care of everything with no hassle and no problem. All I had to do was sign the papers when they were done.
Check with other tire shops that sell Michelin, including those that don't service RV's. They may have forms on hand and can give you what you need to take to a shop you can work with.
Also, be sure to check the spare tire. It may or may not be a Michelin that is covered in the recall. Michelin will replace it, too, if it is in the recalled group.
