Andersen Ultimate Fifth Wheel Hitch setup on new 3500 4x4 Dually

I'm seeking setup advice from anyone with a 3500 dually using the Andersen Ultimate hitch to tow their fifth wheel. Do you have your hitch turned with the ball forward or behind the gooseneck ball?

I have factory 17" wheels and tires with the factory goose neck and puck system. My fifth wheel has 15" wheels and tires.

The ball has three height settings. Which hole do you mount the hitch ball onto the hitch platform. I'm inclined to start out in the middle hitch ball hole but would appreciate any feedback on what would keep the trailer close to level.
