Steve, without pictures of your hitch when assembled I won't even hazard to guess. Here is a photo of how my rig was connected when I towed the travel trailer.

When properly connected the bars should be parallel to the frame. Also look at your hitch head. It should be adjusted up or down to let the camper be level and then canted with the washers to allow the bars to be level to the camper when engaged.

Those brackets don't look like mine either. I went to Equalizer's website and found an owners manual for the 4K pound hitch. Your brackets look just like the ones pictured for the 4K Pound rated hitch. If they installed the 4K hitch it is only rated for 400 lbs tongue weight and 4K lbs trailer weight. Your trailer has a GVWR of almost double that. I think that your hitch (or at least the L-Brackets) are too light for your trailer.

Here is the 4K owners manual (I hope I attached it right)

OK, it would help to take a step back here.

What hitch weight do you have and what is the GVWR of your camper?

I was towing a 6,800 pound camper with a 12,000 pound rated hitch.

Ok all Trudinators DH here.
coupler is style B
hitch is 10k
bolts are tight to frame top and bottom
due to hitch style inverted frame brackets are proper installation
over torqueing of frame bracket bolts is my opinion of "cause of failure"
dealer will install new brackets on 5/16/12 I will suggest this and assist with proper torqueing if they are in agreement with my cause of failure.

thanks to all for ideas and suggestions.
