x2 on working directly with Equal-I-zer. If the bar was secured properly with the pin, I can't think how this would happen other than a material defect in the bar. Get a well-focused closeup of that fracture area to send to them.
The engineers at Equalizer will probably want that bar to analyze, and follow up with their supplier. It is best not to alter the fracture area in any way.
