It is a funnel like devise. You may also need a new ball. The two are around $90. Well worth it. New models come with it.

Yep. Bought mine on Amazon a while back. Well worth the money as it gives you a little larger margin of error when backing under it.

I watched your video and it looks like you have the 5.5ft bed crew cab (mine is 6.6ft double cab). You should reverse the kingpin adapter to gain 8" more clearance. If 4" is enough then also use your 4" offset ball to make it net 4" instead.

That looks like more of a front cap design issue. It looks like that "scoop" design took up at least 6 inches of possible turn radius. It looks neat, but doesn't do you any favors for that 90 degree turn.

Thanks for posting the closeup. I see now you do have the maximum length. Suggest you add the Reese 4" offset ball then. That will give you another 4" back.

Also, you may not be aware that Andersen made a slight change to the design and they now ship what many refer to as the "Ultimate II". It's easy to tell the difference. Yours has a single triangle logo plate that's at the front of the frame and faces the cab when your hitch is installed. The Ultimate II has two triangle plates, one on each side of the frame when installed. The new design includes the funnel ring for easier hitching. But what's most relevant for you is that their design change also moved the top ball about an inch or two further away from the cab.

The Andersen Ultimate is sold with a lifetime warranty. I suggest you contact Jason at Andersen and have him look at your video. Andersen has been willing in the past to swap out for the newer model as a warranty exchange. You may find that a combination of the new design plus a Reese offset ball is all you need to get the 90deg turn.

I use an Andersen Aluminum Ultimate Connection, Andersen Camper leveler, and Andersen Rapid Jack. Andersen makes great products.

Another option for a light weigh, empty bed hitch is the Reese Goosebox. It replaces your entire pin box and you only have the turnover ball in the bed. Lippert approved. It also has suspension and the height is adjustable with the standard pin box bolts and the air suspension fill.
