Jack repair?

Got home from camping this weekend and noticed that the foot of my jack had come off in travel. Does anyone know the recommended procedure? Do I need to replace everything or can I replace just the base. Thanks 2011 Georgetown 337

I have purchased the disc from Lippert. Ordered via phone tell them jack rating and plate diameter. You'll also need the cap screw and the rubber O'ring which is part of the cap screw.
