There's a small flow control orifice where the shower hose connects to the C/H faucet. See if that opening is restricted. Besides that can you connect a small air compressor to the same joint, 20-30 psi is plenty. Open the cold water low point drain and blow the line back wards. There really is no other restrictions to the fresh water piping to the shower. Just on a chance have you taken the faucet apart? Remove the handles and take out the stem assembly. check for calcium build up, but I doubt it.
