slide out of wack when closing

HELP!
I have a 2016 V-lite with three slides. The smallest (bedroom slide) only has one actuator. This slide has been OK for the first year but on our first outing this year my DW was closing it and it caught the door nob on the closed bathroom door and crushed the door.
I spoke with the technician at the dealer I purchased from and he says its a design flaw and it should have come with two actuators (like the other two slides).
Forest River sent me a fix procedure of removing one of the washers from the mounting arm. Dealer tech says it really won't change much. I have unloaded the storage compartment on this slide and it made no difference. I have lubricated the underside if the slide box with white lithium grease, no change.

Anyone else have or had this problem?

I think Lipert? I talked with one of there engineers and they really had nothing but agreed with my rv dealer technician that there really should have been two actuators...

Is this a "Gear Rack type or Schwintek ,Side Drive? They are Both made by Lippert! Youroo!!
