You are correct on the husband" and "wife" part. "D" usually stands for "dear", but could stand for other adjectives, depending on the speaker's (or typist's, in this case) frame of mind.
Similarly, DS = Dear Son and DD = Dear Daughter. Note that you will rarely, if ever, see DM-I-L
