The Service Request form has not been posted yet. FROG Bob is having a meeting today (Monday) with the FR service folks, and pending finalization of some details, the service request will be posted on the FROG website later this week.

You will download it, fill it out, and it will contain instructions on how/where to submit it.

Don't worry, you won't get overlooked. Everybody who is registered for the rally will get a chance to submit a request. This is not "first-come-first-served" like registration was.

Found the service request form, but when I try to print it, the right side of the form is missing.

It printed out correctly on my printer.

Do you have an print option to select "fit one page"? Double check that portrait selected not landscape.
