Open Ground indicated on one side of SSP-50X

Last week we were at a CoE park and I had an open ground indicator on one side of the Progressive Industries SSP-50X. It was Green-Blue on one side and only Green on the other.

What's this mean and should I have not used that site? It was on all three outlets, the 50a, 30 and 20a.

I would say the unit is faulty. If you an open ground both legs would be open not one. Unless I'm missing something
