Yes, I do. When traveling more than a couple of hours from home, I carry an extra spare. Call it overkill or whatever you want, but twice now I've had camping buddies blow out two tires within a few miles of each other, and one blow out two at the same time. Also, my rig is so heavy that if I lose one tire, even with a TPMS, by the time I get stopped, the good tire is going to be well over loaded which can cause significant damage to it. I have a rule that if one tire on the rig blows out, change both at the earliest convenience.
