Ultra-Lube Axles

I greased my bearings through the Ultra-Lube Axle system before my last trip. When I pulled the rubber cover, I found that two of the four Zerk grease fittings were just laying in my grease cap. I didn't see any lose of grease. I screwed them back in and went on with my grease application. I checked the other two and they were tight.

How often should the bearings be greased?
