One of two things. bad connection on the backside of that switch...which is probably not it (that can shut the system down).
Second. (most likely), the guys put a duplicate two button switch in. Each switch has an "address". 001, 002, etc. If they put a switch in by accident that is duplicated elsewhere, it will communicate with only one of them.
