I can't speak specifically about your rig, but for many engines, the fan doesn't engage until you're at RPMs above idle. I had an issue like this in my F-150 and I would need to pop it into neutral and rev the engine from ~600 RPMs in idle to ~1,000 RPMs. This would engage the fan and cool everything down very quickly.
Some people even go as far as to install electric fans with a thermostat ... so it kicks on at a certain temperature, regardless of engine RPM.
