Thanks again Cover Dog for Rhonda's phone number. I finally had time to call her this morning. She was very gracious and as soon as I provided her with a VIN she emailed a copy of my extended warranty paper work.
It was the form I submitted, signed by Forest River and stamped PAID, just as you all said.
I never received the original form back, apparently the post office re-routed it for me.
Rhonda was very helpful, just like everyone here on the forum.
