Sierra axle info help

I have a 2004 forest river 5th wheel sierra. I have been going through tires every single time I go out! I have had wheel lugs shear off, and have even had the rims separate at the weld at times. I have looked at the axle tags, and all it says is:
Model D 50
DATE 229402
TLR 375 in
6000 lb
Hayes axles

my trailer weighs approx 14,000 lb.

any ideas on what is really causing this problem?

Who does your maintenance?

Have you weighed it recently "ready to camp"?

I do most of the maintenance. ASE master tech. but that really doesnt mean anything lol. I keep the brakes, lubes, air pressure, all current and properly adjusted.
Empty, with approx 8 gallons of fuel on board, but with empty water tanks, it weighs in at 13,755 lbs.
the tires currently are load rance E, but when I get a blowout, which is every time I take it out, the other tires are HOT to the touch! I have even broken all of the original leaf springs, but have since replaced them with a heaver duty set.

and by the way, the axle tag says D 60 not the D 50 as I miss typed prior. they are 6000 lb axles

Just a wild thought. Have you checked the "trueness" of the axles? Are they running straight. Blowing tires, shearing lug nuts, tires running hot, sounds like something is not aligned correctly.
