TV bracket pulled away from the wall

When we were setting up after a 60 mile trip my DW noticed that the TV was about to fall to the floor. The mount pulled loose from the wall. It is a 40in Toshiba mounted in the slide out on our 33ckts. Anyone else have this happen?

Thanks to all who replied. The dealer fixed the problem. There were only four screws holding the bracket to the wall. What was the factory thinking? That's not nearly enough to hold a 40 inch TV. The service department reattached it with twelve screws and glue. Judging form the lack of replies I must be the only one to experience this.

Another thing the service tech. did was to make two supports to go under the TV. They were pieces of wood covered in fabric that extended from the back wall to about an inch in front of the TV. They were a thickness that allowed them to just barely fit under the edge of the TV and they were about an inch to an inch and a half wide. Their purpose is to keep the TV from bouncing up and down while traveling . I thought that was a good idea.
