Defective Oil Pan

In March 2016, my wife and I purchased a new Forester 3011DS which is on the Ford E450 chassis. We recently took it in to the local Ford dealer for the first oil change. After they drained the oil and went to put the drain plug back in, the thread part that is welded to the oil pan popped off into the oil pan. To make a long story short, Ford had to order a new replacement oil pan. The service center said this was a first for them and had never seen something like this happen before. Fortunately, everything was covered under warranty. Has anyone else ever had a similar problem with a defective oil pan?

Back when I had my 1977 F150 that same nut split on it. I ended up pulling the pan and welding a pipe fitting on it. Never had a problem with any other since. They are only spot welded on so if they didn't do it right it could happen.
