How to set up a survey?

I scanned around the forum but can’t see how to set up a survey.

Obviously I’ll post it under the appropriate forum subheading.

By “survey”, do you mean a poll? When you open a Post New Thread page, down at the bottom there are additionals option, one of which is to post a poll with the thread.
