2016 Goshen Rally Service Request

I am trying to submit my service request form for the rally in August. I would like to submit the form online, but I can't figure out how to do that. I have the form filled out, but have no way to submit it. So, I thought I could save the document and email it Bob. There is even an option to email the form to Bob at the bottom of the request form. If I try to save the PDF, it does not save any of the information I put in. If I try to do a select all and copy to an email or to a Word document, all I get is unformatted text.

Any ideas would be appreciated.

Never mind. I decided to print the form, scan it and send the scam

Send the scan to Bob. Sorry about the split post. I should know better than to try to type on my tablet.

That's the way I did it from my cell phone. I have a Epson printer scanner and wirelessly sent form to printer, filled it out then scanned it back to my phone and emailed it. You did good.

We had the same problem, even emailed a blank for to Bob. Our solution was the same...print, scan and email.

I printed mine as a pdf then sent it to Bob.

I emailed mine as a .pdf attachment.

I emailed mine as a PDF attachment as well. The first attempt ended up blank as some had mentioned but I opened the file again and filled it out and then saved it and it took the information then.
