lippert electric cable retraction for slideout... diy help.

My cable retraction flexible "arm" is/has broken loose on the slide out end. Lippert (via amazon) sent me a box w/these additional items in it:

Where do they go?, there's no instructions in the box and I already know where the flat plastic flexible arm should go or at least, where it was attached originally (improperly?), but what are these extra parts and how, if at all, should I use them? Also, anyone have a pic of "how" it (the flat plastic cable support) should look in the retracted position?

Lastly, there's been "drag marks" under my slideout since I bought it and I suppose it's a roller not rolling anymore? How would you go about fixing that? Looks like it's hard to get to unless it's accessed from inside via a removed drawer? see pix...
