slide motor falls off

Anyone else have a problem with motor coming off the actuator? Fiver is a 2012 Flagstaff 8528RKWS and problem is with main slide. Trouble began on first trip which happened to be to the factory for a tour. Banging against floor when retraction was complete. Service mgr. repaired and said bolt had loosened on the slide motor. Good for 8-10 cycles and then motor fell off. Fortunately with belly cover there is no place for it to go. A dealer in Billoxi repaired by putting in longer bolt with locking nut run on it. Lasted 2 monthes and fell off again, re-installed by myself. Complaint to dealer resulted in factory installing new actuator and motor assembly last July. Now after a 3 month trip it has fallen off again and I re-installed it again. Calls to dealer, factory, and Lippert (mfg of assembly) have been freindly but no one has heard of this problem. By the way, if like me you can't picture what the actuator is, I believe it is the long tube that the motor and gear box hooks up to.

Reinstall with fine thread bolts and nylon lock nuts

Gr 8 bolts, locknuts & Loctite, or drill the bolt, castellated nut and cotter pin.

If the bolts just go into a tapped hole, drill the head of a gr 5 or gr 8(if you can) and safety wire them.
