I'm curious what the problem turns out to be in the tank?
As I speculated earlier, my guess is dip tube missing, broken or not positioned to draw gas....and liquid flow will quickly cause regulator problems since they typically are meant for just gas use.

Update it looks like it was the propane tank. I installed a new today and everything seems to be fine.
