Wheel failure

HI all I am new to the forum and I was wondering if anyone has had issues with their rims breaking? I had a catastrophic failure this week were I had the rim and tire come off with the center piece stayed on the cat. It destroyed all the sheet metal on the cat but my concern was if this had happened at speed or on a cure it could have injured the family or worse. It also could have injured or worse an innocent person really twisted my stomach. I had a less destructive failure a few years ago where we just lost air when a weld broke pulling into my site. I get tire failure it's part of trailer life. Any experience anything like this. It's an o6 31qbh we'll maintained but has had it's share of the factory demons below is a link of what I am talking about

First time I see this, had to have a lot of stress on the rim to fail at the weld.....

FONZIE the sad thing is the camper was practically empty. We were staying at a family members house so we had very little packed and about 10 gal of water in the tank. I posted this on another site and had a few come forward with wheels having issues like cracked welds were they woke up with flat tires. Makes you wonder..
