Identifying Leveling Jacks

I have damaged one of my leveling jacks on my Wildwood T25KS. I want to purchase an exact replacement so I can use the undamaged welded on mounting brackets. PROBLEM... there is nothing on the jacks to indicate who they were manufactured by.. The only thing is the sticker that is shown in the attached file. I have tried to find the Model J-5000 number but have came up with nothing.. I believe that the J-5000 indicates it is a 5000 lb capacity jack.. can anyone shed any light on what brand of jack this might be from the label picture..
