Look for the manufactures name. If no name is available, post a picture and surely someone on the forum will be able to identify if for you.

Usually a very easy fix, I just did one of mine after a hail storm knocked one out. Mine was a ventline, it is on a very simple metal hinge. On the inside remove the screen and loosen or remove any attachments that are required to get the metal arm out of the brace in the top that raises it, then on the outside on either end of the hinge you can bend a small metal tab up and the lid slides right off. The whole deal took all of about 20 mins and that includes getting the ladder out of the garage and putting it away.
