Sounds like the gate valve wasn't closed all the way. Maybe a chunk of TP, or something else, got in the way when it was last closed preventing it from sealing up all the way. Or it could be a faulty seal.

Sounds like one or both blade valves are leaking, very easy to replace both.

Has it happened more than once. Could it be someone pulled the handle and then closed it with the cap still on? Supposedly "jokesters" find it funny to do.

It's not uncommon for a gate valve to leak due to blockage or not closing completely. Ours uses a cable and sometimes won't close all the way. Lubrication helps, just have not taken time to remove the underbelly and disassemble the valve. In the meantime, I put a small bucket (plastic trash can) under the outlet to catch any leakage when I take the cap off.

Mine ('05 FR Grand Surveyor) uses the cables also. Personally I think they should outlaw these. One problem I found was that when closing the valves the cable housing would "buck up" and not allow the valve to completely close. It took a few hours of watching how this works to determine that an additional cable housing support clip was needed to hold the housing in a straight line so the cable would push the valve closed. I finally did get it much better and have not had the "surprise mess" when taking the cap off.
